Where a hospital system runs a single facility, a Health Information Management System operates at district, provincial and national level, aggregating data from every facility into a coherent picture of the health sector. It standardises the data set each facility reports, receives submissions from health centres, aid posts and hospitals — including offline and mobile capture for facilities without reliable connectivity — and validates them for completeness and plausibility before aggregation. Core modules cover disease surveillance and outbreak alerting, immunisation and maternal-child health programmes, medical supply and cold-chain stock visibility, human resources for health, and facility registries. Outputs are the indicator dashboards and statutory returns used for resource allocation, programme monitoring and international reporting obligations, replacing paper returns that arrive late and inconsistently compiled.
